Letter
of Request |
Submit
a letter requesting a Certificate of Need and
a Solid Waste Management Permit. The letter
must state your company's type of operation
and include your Federal Identification Number.

Permit
Application Fee |
Enclose
a check for the $150.00 Permit Application Fee
payable to the Board of County Commissioners.

Original
Certificate of Insurance |
The
Certificate of Insurance (COI) is a list of
insurance policies and coverages, including
Workers' Compensation coverage. Note
that the Board of County Commissioners must
be listed as Additional Insured and the COI
must include 30 days written notice of cancellation
on the certificate. Refer to the Escambia
County Resolution R2006-169 to view
the insurance requirements for a Solid Waste
Permit. Sample Certificate of Insurance. *Please see the note below
